Latest medical report from the treating hospital, including observations, ventilator settings, drug infusions and imaging reports. Discharge summary if available.

Next-of-kin or patient consent for transport. Insurance authorisation if cover is claimed.

Airline

MEDIF form for commercial stretcher or escort cases.

Receiving hospital
