Patient identity and travel
Passport and any visa documents. Death certificates and embalming paperwork for repatriation of remains (a separate service).
Medical
Latest medical report from the treating hospital, including observations, ventilator settings, drug infusions and imaging reports. Discharge summary if available.
Authorisations
Next-of-kin or patient consent for transport. Insurance authorisation if cover is claimed.
Airline
MEDIF form for commercial stretcher or escort cases.
Receiving hospital
Confirmation of bed availability and consultant acceptance.
